Photographic Consent & Release Form
1. Consent to Photograph and Record
I, the undersigned, hereby grant MIA Elite Academy and its representatives, agents, employees, and contractors the permission to photograph, record, or videotape the Participant during participation in MIA Elite Academy’s programs, events, and activities (the “Activities”).
2. Use of Photographs and Videos
I understand that the photographs and videos may be used by MIA Elite Academy for promotional and marketing purposes, including but not limited to, publication on the academy’s website, social media accounts, print materials, and other promotional channels. This use may include the Participant’s image, voice, likeness, and other identifiable characteristics.
3. Waiver of Compensation
I acknowledge and agree that I shall not receive any financial compensation, royalties, or other forms of payment for the use of photographs, videos, or recordings of the Participant.
4. No Obligation to Use
I understand that MIA Elite Academy is under no obligation to use the photographs or videos and that there is no guarantee that the Participant’s image or likeness will appear in any specific materials or media.
5. Duration of Consent
I understand that this consent is granted in perpetuity, meaning that MIA Elite Academy may use the photographs or videos in the future without requiring additional consent from me.
6. Revocation of Consent
I understand that I have the right to revoke this consent at any time by submitting a written request to MIA Elite Academy. Upon receipt of my request, MIA Elite Academy will cease using any new photographs, videos, or recordings from the date of my revocation. However, revocation will not affect the use of materials that have already been published or used prior to the revocation.
7. Acknowledgment
I acknowledge that I have read and fully understand this Photographic Consent Form and that I am signing it voluntarily. If the Participant is under 18 years old, I, as the legal guardian, am signing on their behalf and accepting the terms on their behalf.
